Clinical and Histopathological Study of Adenoid Cystic Carcinoma Occurring in the Salivary Gland.
The First Report: Study of Local Recurrence

Fifteen patients who had been treated for adenoid cystic carcinomas in our department during the past 22 years, from July 1971 to July 1993, were studied to determine the degree of local recurrence of the carcinomas and to identify related factors. The results were as follows: i) Local recurrence was identified in 8 (53.3%)of 15 cases. The length of time from operation to local recurrence ranged from 3 months to 9 years and 1 month. ii) The rate of recurrence was found to be 33.3% and 66.7% in lesions of the major salivary gland and minor salivary gland, respectively. The length of time to local recurrence was generally short in the former cases, but in the latter cases relapses generally occurred more than 6 years after the operation. iii) There was no correlation between the rate of recurrence and T classification or the clinical stage. However, the period of time to local recurrence was relatively short in progressed cases of T classification and the clinical stage. iv) The rates of recurrence were 85.7% in cases featuring local extension (T1-4b), and 25.0% in cases featuring no local extension (T1-4a). The former cases relapsed earlier, but the latter cases mostly took more than 6 years to relapse. v) Regarding the rate of recurrence by histopathological classification, it was found to be higher in types I and II. However, in types II and III the tumor tended to relapse earlier. vi) The rate of recurrence was 37.5% in cases of complete removal of tumors, and 83.3% in cases of incomplete removal. The latter cases tended to relapse soon after the operation. '
